Forefoot Varus or Supinatus
The forefoot varus versus supinatus is a widely debated topic, and is mostly due to misinterpretation of the definition.
A Forefoot Varus is a congenital osseous deformity in which the forefoot is relatively inverted to the rearfoot. This deformity can be seen when the subtalar joint is helt in the neutral position and the midtarsal joint is maximally pronated (MCPP).
A Forefoot Supinatus is an acquired soft tissue adaptation of the forefoot in an inverted or supinated position around the longitudinal axis of the midtarsal joint.
A Forefoot Varus induces STJ pronation whereas a Forefoot Supinatus is created because of STJ pronation. The adaptation of the soft tissue can be explained by Davis’s Law.
In ‘97 Thomas Michaud wrote this short article on this topic
Some good discussions on this topic on podtiatry arena: 1
In therapy you should be aware if you are confronted with a Forefoot Varus or Supinatus because they need a different approach!
Wedging a Forefoot Varus , when actually being a supinatus (being most of the time), will increase the problem. Intrinsic orthotic correction and control on the STJ pronation will be the best approach to this problem.
Testing your shoes
In addition to my previous post on shoe quality, I found thos video that shows what you need to do.

piezogenic pedal papules
It’s been TOO long since I wrote on my blog, but due dates for my 2 research papers ask all my attention. With one turned in and one almost finished and ready to go – I have a few moments for some ramblings.
I’m happy to learn every day something new. The fantastic advantage of working together with colleagues is that they bring new knowledge without really being thought.
My new word for today is Piezogenic Papules, never heard of this before, but I definitely knew the clinical signs. and NO, it’s not contagious!
Tnx PJ, it’s always a pleasure teaching with you!
Mitsubishi Outlander
As mentioned earlier in the ‘my-hot-wheels’ posting, my newest aquisition has arrived.
The Mitsubishi Outlander Intense Plus is my new toy bringing me in normal 2 wheel drive or 4×4 to wherever I need to be.
Driving away on cruise control listening to the 650watts Rockford-Fosgate stereo, the smell of new ‘cognac’ colored leather seats and a nicely fully automated air conditioned interior, brings a smile to my face.
Already tried out the extra third row in the back with my son and Antje, both being under 5 year old, they were having a ball!
If you’re up for a ride, let me know!
